NEW TRANSPORTATION BROKER COMING APRIL 1, 2026!
Inclusa is moving forward with a partnership involving a transportation broker, Medical Transportation Management, better known as MTM. We are currently in the process of transitioning services to this new support model. Read on to learn more!
Who is Medical Transportation Management (MTM) Health?
MTM is a transportation management company specializing in non-emergency medical transportation (NEMT) who serving Medicaid, Medicare, government agencies, and managed care organizations for 30 years. MTM partners with organizations to support their membership with accessing healthcare, increasing independence, and connecting community resources in the most cost-effective manner.
When will the switch to MTM occur?
Inclusa transportation contracts will end at midnight on 03/31/2026. MTM begins service on 04/01/2026.
What does this mean for providers?
Transportation provider contracts with Inclusa will be ended effective midnight on March 31st, 2026. Letters have been mailed to impacted providers regarding the transition to MTM and next steps.

What does this mean for members?
Members will continue to receive transportation services as authorized by their Care Teams. The coordination of services and the management of transportation providers will be directed by MTM. Members will have the opportunity to easily navigate and request their transportation via website, mobile apps, and online portal.
Why is this change occurring?
Inclusa and iCare operated utilizing two separate transportation service models, it became essential to align transportation service models. After reviewing internal and external solutions, MTM was selected to streamline processes and enhance member support.
How does my company begin the process of working with MTM?
Providers interested in supporting Inclusa members with transportation services should reach out directly to MTM to initiate the onboarding process. MTM will guide providers through required discussions related to credentialing, service expectations, and next steps for establishing a working relationship.

As you know, the assets of Inclusa were acquired by Independent Care Health Plan, a wholly owned subsidiary of Humana and as a result, Inclusa has transitioned to a new email domain. Inclusa email addresses were updated from @inclusa.org to @inclusa.com.
Reminder of what to expect for the Inclusa email domain change:
- Email is now from @inclusa.com instead of @inclusa.org.
- To ensure continued delivery of Inclusa emails, please work with your IT department to add our new domain to your allowlist.
- Emails sent to @inclusa.org addresses will be automatically forwarded to the new @inclusa.com addresses.
- Inclusa’s name, website, and physical address remain unchanged.
We are also sharing the Secure Email Guide for Recipients to assist you with any secured emails you might receive from an associate affiliated with the following email domains:
- inclusa.com
- icarehealthplan.org
- humana.com

Apply with Medicaid by December 31.
All providers of Medicaid waiver authorized services must submit an application to enroll or revalidate using the ForwardHealth Portal immediately. This will allow you to get paid for dates of service on and after April 1, 2026.
If you don’t apply, your contract(s) with Inclusa/iCare will end, and you will lose business from people covered by Medicaid.
Please Note: Some providers may have to enroll for each location where they provide services.

What to expect:
Email will begin arriving from @inclusa.com instead of inclusa.org.
Emails sent to @inclusa.org addresses will be automatically forwarded to the new @inclusa.com addresses.
Inclusa’s name, website, and physical address remain unchanged.
To ensure continued delivery of Inclusa emails, please work with your IT department to add our new domain to your allowlist.
ATTENTION ALL PROVIDERS: Important Enrollment Requirement – Action Needed by January 1, 2026
Effective January 1, 2026, all providers must obtain a Medicaid ID number by enrolling with Wisconsin Medicaid through the ForwardHealth Portal.
Key Points:
- Enrollment is required to continue providing services and receiving payment from Managed Care Organizations (MCOs) or IRIS (Include, Respect, I Self-Direct).
- Some providers may need to enroll separately for each service location.
Providers who are not enrolled and do not have a Medicaid ID by the deadline will not be eligible to provide services or receive payment.
