Inclusa utilizes the transportation broker Medical Transportation Management (MTM) to support our members’ transportation needs.
What is a Transportation Broker?
A transportation broker is a company that helps people get rides to their non-emergency medical appointments or to other community locations.
How does it work?
Your Inclusa Care Team approves and authorizes your transportation based on your individual needs.
-Usually, your Care Team will authorize transportation for a period of time that matches your care plan, for example, a 6-month period.
-Once transportation is authorized, you will work directly with MTM Health (the transportation broker) to help schedule and organize the rides for you.
-MTM Health will make sure you get picked up and taken where you need to go.
-MTM Health has providers across the state, and various transportation options such as gas mileage reimbursement, bus passes, Rideshare, and traditional transportation including wheelchair accessible vehicles.
How would I schedule transportation with MTM?
–As soon as you know you need a ride, please call MTM to schedule it. It is encouraged to call at least 48 hours prior to needing transportation to ensure successful set up
–Call 888-889-0305 (TTY: 711) 24/7, including holidays
–Use the free MTM Link Membermobile app on your cellphone. Search “MTM Link Member” in the App Store or Google Play (user guide: Mobile App User Guide)

MTM is the transportation broker for Inclusa/iCare members, but they are also the transportation broker for the Wisconsin Medicaid NEMT program. These transportation benefits are separate and distinct programs.
