A transportation broker is a company that helps people get rides to their non-emergency medical appointments or to other community locations.
What if I am an interested Transportation Provider looking to contract with MTM, who can I contact?

What if I am a contracted Residential Provider with Inclusa and want to learn more about how I can set up rides on behalf of Inclusa members through MTM Link Portal?
You can call the same tollfree number- (888) 889-0305 that our members use or create an account in the MTM Link Facility Portal by submitting a New MTM Link Facility Portal Registration Request.
What if I am contracted with MTM to provide transportation but have a concern about how the partnership works?

Be sure when outreaching about your Inclusa MTM rides, you use the contact information located on this webpage to ensure you are accessing the appropriate MTM staff to support you. MTM is the transportation broker for Inclusa members, but they are also the transportation broker for the Wisconsin Medicaid NEMT program. These transportation benefits are separate and distinct programs. Each program has different MTM phone numbers, policies, procedures, eligibility requirements, provider requirements, and payment processes so it is important you use the contact information located on this webpage.
