Commonunity® is Inclusa.

More specifically, Commonunity® is Inclusa’s approach to everything we do. So central to our operations, Commonunity® is a trademarked model of managed care that places emphasis on the importance of full citizenship and what it means to be an active participant in everyday life. Explored through both a collective and individual approach, Commonunity® in its purest forms is described as follows:

Commonunity® is the belief in the strengths of everyone and the commitment to support the common good for all.

What this means for the common good:

  • Partnerships with others.
  • It’s about possibilities.
  • Strengths of all enrich community.
  • It’s a belief in better.

Commonunity® is a way of living which brings together the basics—connection to community, opportunity to work, a place to call “home,” ability to get where I need to go—to create the life of my choosing.

What this means for the individual:

  • I live the life I choose.
  • I have relationships in my community.
  • I feel like I belong.
  • I work and contribute.
  • I have access to my community.
  • I have meaningful connections.
  • I am valued.
  • I choose where I live.
  • I am responsible.
  • I am a citizen.

In every day practice, Commonunity® explores the importance of full citizenship by supporting the development of meaningful connections between members and their communities through the thoughtful exploration of five component areas: Community Connections, Self-Determination, Community Living, Integrated Employment, and Mobility.

Commonunity® Components

Full citizenship includes many components, and can look different for each individual. The core components of Commonunity are about the basics of what we each desire from life; yet, not everyone has had the opportunity to explore, let alone embrace, the basics of what makes life so full. In order to support citizenship while remaining flexible to this individualized approach, five components are central to Commonunity®.

Community Connections

The whole is greater than the sum of its parts.

Community is a place where all should belong. Communities are strongest when banded together, and that includes supporting each and every member within. But what if the whole didn’t embrace all of the parts?

Unfortunately, not everyone has the opportunity to explore community connections. At CCCW, we want to change that.
Commonunity® is built from the belief that everyone deserves the right to contribute to community and actively participate in full citizenship. Community connections foster unity in the common good for all where everyone can consider: How can I be part of the whole?

Self-Determination

You can do anything you want to do. You can be anything you want to be.

Self-Determination is a theory of motivation which focuses on individuals having control over their lives and exercising the basic rights of citizenship, freedom, liberty, and the pursuit of happiness. But what if those basic rights were not a reality for us all? What if you instead heard: You can’t do anything you want to do. You can’t be anything you want to be.

Unfortunately, not everyone has the opportunity to explore a Self-Determined life. At CCCW, we want to change that.
Commonunity® is built from the belief that everyone deserves the right to control his/her life and actively participate in full citizenship. A Self-Determined life fosters unity in the common good for all where everyone can consider: I can do anything I want to. I can be anything I want to be.

Community Living

Home is where the heart is.

Home is more than a place of residence. It is a place you call your own, where you are safe, comfortable, supported, and able to be yourself. But what if no part of your home was your own; would it still have your heart?

Unfortunately, not everyone has the opportunity to explore community living. At CCCW, we want to change that.
Commonunity® is built from the belief that everyone deserves the right to embrace the ideals of home and actively participate in full citizenship. Community living fosters unity in the common good for all where everyone can consider: Home is where my heart is.

Integrated Employment

What do you want to be?

Employment is part of the human experience and one we have been preparing for since childhood when asked this very question. But what if you hadn’t been asked and instead of limitless possibilities, your life was built from possibility limits?

Unfortunately, not everyone has the opportunity to explore employment possibilities. At CCCW, we want to change that.
Commonunity® is built from the belief that everyone deserves the right to pursue employment opportunities and actively participate in full citizenship. Employment fosters unity in the common good for all where everyone can consider: What do I want to be?

Mobility

Take the road less traveled.

The ability to take the road less traveled is a choice all individuals deserve to explore. But what if the road less traveled, was instead the road never traveled due to limited mobility and minimal access to transportation?

Unfortunately, not everyone has the opportunity to explore mobility and transportation options. At CCCW, we want to change that.
Commonunity® is built from the belief that everyone deserves the right to access his/her community and actively participate in full citizenship. Mobility fosters unity in the common good for all where everyone can consider: Which road do I want to travel?
When individually defined, and collectively supported, Community Connections, Self-Determination, Community Living, Integrated Employment, and Mobility supports everyone to live the life of one’s choosing where opportunities become the practice, and possibilities become the standard.
