Contract – Applications

In June 2023, Inclusa entered into an Asset Purchase Agreement with Independent Care Health Plan (‘iCare”), a Humana Inc. subsidiary, pursuant to which iCare acquired substantially all Inclusa’s Family Care assets. iCare is working toward system integration; however, current iCare and Family Care branded Inclusa are operationally independent.

All providers interested in participating in the Family Care (branded as Inclusa) and Family Care Partnership (iCare) programs must complete an application and meet all contracting requirements including execution of a Service Agreement.

NOTICE: Important Application Update

iCare is temporarily pausing on accepting new applications for residential providers to join our Family Care (branded “Inclusa”) and Family Care Partnership (branded “iCare”) network during the time period of August 18 through September 26, 2025. We will resume accepting new applications after September 26th. We value our provider partners and always encourage high quality providers to apply to join our network. We strive to provide timely turn-round and communication to providers seeking to join our network so this brief pause will allow us to stay timely with processing the applications already in process. We appreciate your partnership to serve our members and look forward to accepting new applications again in the near future.
